This page contains the forms and information that you will need in the effort to complete your benefits enrollment process. Please click on the links you see below to download the files to your computer.
Document Folder Links
Benefits Forms for FGP and Housestaff– This folder has all of the benefits forms that a new hire would need to complete to enroll in the group benefits plans. You will need to print these forms, fill them out by hand, then send them to our office. Be sure to do this before the end of your one-month enrollment window.
Information for FGP and Housestaff– This folder contains legal benefits documents that you are required to receive, such as the Marketplace Notice. It also includes information you will need to make important benefits decisions, such as rate sheets and benefits descriptions. You may view these documents on-line and/or print them. You may also request printed copies from our office.
Payroll Signup Forms – This folder has all of the forms new hires must complete for their appointments, such as tax withholding forms, I-9, and direct deposit. These forms should be completed on or before your first day, but no later than your third day. Please know that your benefits elections cannot be processed until these payroll forms are completed.
Additional New Hire Training Requirements-This folder contains additional information for new hires. In this folder, you will find information regarding Instructions for Departments and Faculty for the Online Orientation Modules and ID badge information.
