Benefits Open Enrollment for 2015

Open Enrollment period is November 1 to December 1, 2014. Be sure to get your benefit changes in by the deadline.
During Open Enrollment you may enroll in Medical, Dental, Vision, Flexible Spending Accounts, Optional Life (up to 3x salary), Dependent Life and Optional Long Term Disability benefit plans. Changes take effect Jan. 1, 2015.
If you’re satisfied with your current benefit elections, you don’t need to do anything except re-enroll in a Flexible Spending Account (FSAs require annual enrollment).

Open Enrollment Forms
Select the form below for the benefit plan you wish to update. Print it, fill it out, then send to our office for receipt by 12-1-2014. You can send your form:
IN PERSON: visit our office on 4C of the Central Building. Employees at ACH may visit the UAMS Pediatrics Office in the Sturgis Building, 4th floor, Suite 4175.
BY FAX to (501) 686-5386. Our fax machine will be very busy. Make sure it transmits by printing a fax confirmation and keep it with your original.
BY E-MAIL: scan your form and send to AskHR@uams.edu. Only do this if you are sending from an internal UAMS email address so your document is secure.
BY CAMPUS MAIL: address to HR Benefits, box 564. Be sure to keep a copy and send far enough in advance so that our office receives your form by Dec. 1.
Medical: enroll, add or drop family members, change between Point of Service and Classic plans, or drop coverage altogether
Dental: enroll, add or drop family members, or drop coverage altogether
Vision Add or Change: enroll for the first time, add or drop family members (but you remain covered), or change between Basic and Expanded plans.
Vision Drop: use this form if you want to drop all coverage
Premium Taxation: change how Medical, Dental or Vision premiums are deducted from your paycheck, either before-tax or after-tax
Life, Long Term Disability, AD&D, Beneficiary Change: buy-up your life and LTD coverage, enrolled in Accidental Death & Dismemberment coverage, or update your life insurance beneficiaries. Click here to see the benefit highlights for these plans.

Talk to your benefit vendors in person at the Employee Benefits Fair on Wed, Nov 19, from 9am to 3pm on the ground floor Ed II. If you are located at Ark. Children’s Hospital, attend a mini-fair sponsored by the Pediatrics department on Thurs, Nov 20, from 9am to 3pm in the Children’s Hall.
