Accidental Death & Dismemberment
Accidental Death and Dismemberment insurance, or “AD&D”, pays a benefit to your beneficiary in the event of your accidental death. It also pays a partial benefit to you should you lose sight or a limb. Our plan is a fully-insured group plan, underwritten by The Standard Insurance Company. Coverage amounts are available in $25,000 increments, up to a maximum of $300,000.
Once you elect your amount of coverage, you would choose between “employee only” or “family” coverage. If you elect family coverage, your spouse is covered at 60% of your covered amount; eligible children are covered at 20% of your covered amount up to $25,000. Eligible children are those under age 26.

What does it cost?
Your premium cost depends on the level of coverage you select, and whether you are enrolling in coverage for just yourself or for your family. The maximum monthly cost – for $300,000 family AD&D coverage – is $9. Rates for part-time employees are the same as for full-time employees.
How do I enroll?
You can enroll, change or drop your AD&D coverage at any time. Your election will take effect the first of the following month. Forms are available in our office and at Benefit Forms.
