What is a qualified office visit?

IMPORTANT:  the relaxed criteria described below only applied through October 2018 and qualified you for the 2019 wellness reward (along with a successful tobacco pledge). 

The originally communicated requirement of getting an annual wellness exam/physical has resumed.  Be sure to get an annual physical within the dates 11-1-2018 and 10-31-2019 so that you will be eligible for the 2020 wellness reward.  You can visit any in-network primary care provider for this visit.  Make sure when you make the appointment that you specify it is the annual preventive wellness exam, which is covered in full under our medical insurance.   For women, your annual wellness visit to your gynecologist will count.

For the 2019 wellness reward, a qualified office visit includes preventive exams or screenings, a visit for treatment of a condition or diagnosis, or a visit to manage a chronic disease.  This visit must be between November 1, 2017 and November 1, 2018 (both dates inclusive).

A qualified visit includes any office visit received from an In-Network Primary Care Provider (Family, General, Geriatrics or Internal Medicine) or Specialist (examples:   Cardiologist, OB/GYN, Oncologist, Gastroenterologist, Endocrinologist, Hematologist, Pulmonologist, Nephrologist, Infectious Disease Specialist, Neurologist or Rheumatologist).

A visit to these providers are excluded and do not qualify for step 1 of the Wellness Incentive:  Dermatologist, Allergist, Podiatrist, Ophthalmologist or Optometrist.

There is no form to complete and no need to contact UMR to report your visit. UMR will use claims data to confirm your visit.
